Witnessing a Statutory Declaration

As per the Oaths, Affidavits and Statutory Declarations Act 2005 the Cheif Executive Officer (CEO) can witness a statutory declaration.
  • The declaration must be in the approved format outlined in Schedule 1 of the Oaths Affidavits and Statutory Declarations Act 2005 (WA).
  • The declaration must be fully completed and signed.
  • Any alteration, such as an insertion or erasure, must be initialled.
  • A statutory declaration relating to a law of the Commonwealth must be made on the approved form under the Statutory Declarations Act 1959 (a Commonwealth Act).

Certifying Documents

As per the Oaths, Affidavits and Statutory Declarations Act 2005 the Cheif Executive Officer (CEO) can certify a document as a true copy, provided the following: 

  • Must be an identical copy of the original .
  • Document must be in English unless you can be sure that the original and the copy are identical.
